Here's how:

    1. Purchase an FMT Pro Jig from your US or Canadian Dealer between November 1 and November 30, 2019. List of dealers
    2. Find the serial number on your jig, located on the decal on the back of the jig body.
    3. Cut out the UPC code from the FMT Pro box. UPC code example
    4. Complete the form. Please print clearly. Incomplete or unclear forms will be returned. Click here to download and print the form.
    5. Mail the form, along with the UPC code and original receipt to: Leigh Industries Ltd. Box 357, Port Coquitlam, BC V3C 4K6 Canada
    6. Submissions must be postmarked no later than December 31, 2019.  Please be sure to have sufficient postage.
    7. Leigh will contact you to complete your order of the ACMTC Accessory Kit**. Payment can be made by VISA or Mastercard.
