  1. Purchase an FMT Pro jig from your dealer between Nov 1 and Nov 30, 2022.** (USA & Canada only) Click here for a list of dealers.
  2. Locate the serial number on the back of your new jig.
  3. Complete the form. Please print clearly. Incomplete or unclear forms will be returned. Click here to download and print the form.
  4. Cut out the UPC code from the FMT Pro carton. (A barcode has black bars and 12-digit number).

  6. Leigh will contact you to complete your order for the ACMTC Accessory Kit***. Payment can be made by VISA or MasterCard.
  7. Submissions must be postmarked no later than December 31, 2022. Please have sufficient postage for mailing to Canada.
  8. Reminder: Keep a copy of your UPC code and FMT Pro receipt for your records.
